Lock functions

- Add simplified, better commented config file (ublox_ppp.conf) that relies on defaults for most settings
- Update version to b33c

- Change default rinex version for convbin from 2.11 to 3.03
- Change config defaults to more typical use case values

- Move RTKCONV trace file to output file location and change name to match output file name
- Minor fixes, improvements, and cleanup

- Increase default valid carrier phase threshold from 5 to 7

- clean up recently introduced changes for ambiguity resolution to better track number of ambiguities in previous epoch

- Add a couple more Embarcadero project file updates

- Add support for u-blox VALSET command.  This enables configuration of the F9P receiver from the command file.  Thanks to Nagarjun Redla for providing this code.

- Misc minor source file cleanup

- Replace BDS 2I,2Q,2X to 1I,1Q,1X codes from RTCM3 decoding to match RTCM3.1 spec (also see last check-in)
- Change GPS L2C(M) code for u-blox F9P from L2M to L2S to match Rinex spec

- For code efficiency, E5b is included in the L2 computations, E5a in L5

- Remove C++ style comment to avoid linux build error

- Improve format conversion to RTCM3 for GLONASS satellites by using freq info from MSM7 messages when navigation data not available
- Set Galileo bit in RTCM 1005 message to be consistent with GPS and Glonass
- Update version to b33a

- Fix comment for code table

- Change outlier threshold to apply directly to phase instead of code observations.  For code observations, an adjustment from the phase thereshold is used
- Add freq option "l1+l2+e5b" as equal to "l1+l2" for compatibility with previous version of demo5 code
