Fix realsense2 description xacro

Add xacro missing tags
Update xacros with use_nominal_extrinsics as parameter, according to realsense2_description#f7cd5fced531551ac0e1e449e800fa11acac3944
Added name parameter to realsense.xacro. Also Added first parameter to include or not the model xacro. Added a xacro for each realsense model
